Benefit-Risk Assessment

A systematic evaluation comparing the probable benefits of a medical device against its probable risks. Both FDA and EU MDR require that the benefits outweigh residual risks for all device classes. This assessment informs classification, approval, and labeling decisions.

Benefit-risk determination weighs clinical benefits against residual risks and uncertainties. It is central to FDA PMA/De Novo decisions and EU MDR GSPR/clinical evaluation conclusions.

PROCESS / STEPS

  1. Quantify benefits (clinical outcomes, claims) with appropriate evidence.
  2. Characterize residual risks after mitigations and probability/severity.
  3. Address uncertainty, alternatives, and state of the art.
  4. Document decision logic for regulators and update with PMS data.

EXAMPLES

  • A high-risk implant may be favorable if life-saving benefit outweighs known complication rates with informed consent and monitoring.
